Athletic Policy & Physical Form

Students wishing to participate in fall sports at Northridge Middle School need to make sure that they meet all the requirements necessary to begin practice. A physical examination, code of conduct, insurance, sudden cardiac arrest, and concussion form must be on file in the Northridge office before anyone may participate in any practice.  The physical must have taken place and be dated after April 1, 2016 to be valid.  All the required forms are in one packet, which is available at the NMS office or online at www.mcsin-k12.org, by clicking on Schools, then Northridge Middle School, and then Athletics.  Only the IHSAA physical form will be accepted.  Physicals must be turned into the main office before the first day of practice.  If we do not have a physical on file, athletes will only be able to watch.  We strongly encourage you to turn in all paperwork during the following office hours:

July 18 - July 21   7:30 am - 1:00 pm
July 25 - July 29   7:30 am - 11:00 am

Transportation Fee
All student-athletes participating in a traveling sport must pay a $35 transportation fee per athletic season.  Transportation fees should be given to coaches of the sport in season before the first contest (coaches will set the deadline for turning in the money).  Do not send transportation money to the middle school office.  A cap of $70 per student and $140 per family exists per school year per building.  The accrual toward the cap total is for athletics only (does not include band, choir, or academic teams fees).

Cheerleading - for 7th and 8th grade girls (squads already determined)
Practice for cheerleaders will begin on a date to be announced.

Cross Country - for 6th, 7th, and 8th grade boys and girls.
Practice for both boys and girls begins on Wednesday, August 10th and athletes should meet in the cafeteria right after school.  Practice will be done at 5:15.

Volleyball - for 7th and 8th grade girls.
Tryouts for the team will start on Tuesday, August 2nd.  Practices will be from 3:30 pm to 5:30pm.  Girls interested in participating should meet at the Northridge Middle School field house in time for practice.

Soccer - for 7th and 8th grade boys and girls.
Tryouts for the girls' teams will start on Monday, August 1st. Tryouts for the boys' teams will start on Tuesday, August 9th.  There will be a varsity and JV team if numbers allow.  Boys' practices will be from 4:00 pm to 5:30 pm, and girls will practice from 6:00 pm to 8:00 pm until school starts.

Football - for 7th and 8th grade boys
7th and 8th grade football players will receive equipment on Monday, July 11 from 4 pm to 4:45 pm, followed by skills camp at NHS until 7:00.  Please make arrangements to receive equipment during this time if possible.  Practice starts on Tuesday, August 2nd from 3:30 - 5:30 pm.  Participants should meet at the locker room doors in the athletic hallway of Northridge Middle School.

6th Grade Students
6th graders will have the opportunity to participate competitively in cross country, wrestling, swimming, and golf.

All-Sport Tickets/Athletic Admission
Northridge Middle School will be offering All-sport passes for the 2015-2016 school year.  Passes will cost $75 and will be valid for parents and children of a given family.  Each parent will receive a credit card size pass.  The pass will be good for all Northridge Middle School home sporting events, with the exception of tournaments.  The passes may be purchased during the August registration dates or anytime after.  It really is a fantastic deal if you have a child in at least two sports.  Gate prices for normal admission are $3 for adults and $2 for students and senior citizens.  These tickets are NOT good at High School Sporting Events.  Also, High School All-Sports Tickets are not redeemable at Northridge Middle School Athletic Events.
